Position Description

At Suncoast Hospice, the Palliative Care PFCC (Patient Family Care Coordinator) utilizes management skills and is responsible for identifying and coordinating patient/family needs and for providing palliative, preventative, and rehabilitative care to the patient/family unit. The PFCC in collaboration with the Director of Palliative Care is responsible for agency adherence to conditions of participation and state hospice and home care regulations. The Palliative Care PFCC participates in the agency's strategic initiatives to increase access to the care programs and the development of new programs within Palliative Care.

Position Requirements

1 Current Florida Registered Nurse License.
2 Two years Hospice experience and/or two years Home Health Experience preferred.
3. Two years management experience with supervisory responsibilities, preferred.
4. Seven to ten total year’s experience.
5. BSN or Masters Degree preferred.
